Ghizlane Mutahir, 41, died at Fly Emotion Park in Bema, Italy. The woman was sliding along a cable over the Bitto Valley, at the end of the path she slipped out of her safety harness and fell from a height of 20 m.
An ambulance rushed in, but the woman was already dead. The police and prosecutor's office began an investigation into "murder by negligence."
Mutahir's two nieces, who had just completed a zipline ride, filmed their aunt racing down the steel rope at 96 km/h. Investigators are studying video recordings of the girls.
The tragedy occurred last Sunday when hundreds of people gathered at Fly Emotion Park.
According to eyewitnesses, the woman was in an upright position for several seconds before the fall. She "stopped, began to struggle, and then her legs slipped out."
"She fell from a height of 20 meters into the thick of the forest and died instantly. She may have had a panic attack and that is what made her struggle, perhaps another medical issue, but we are waiting for the results of the autopsy," a police spokesman said.
Prosecutor Stefano Latorre visited the scene and said he had opened an investigation into "murder by negligence."
Authorities will inspect safety equipment and have temporarily closed the park as a precaution.
Ghizlan Mutahir lived in the neighboring commune of Sant'Angelo Lodigiano and was married.
Her husband Michele, who stayed at home with a toothache, rushed to the park and told La Repubblica: "I shouldn't have left her alone. I thought she was just hurt, but she's no longer with us."
Patrizio Del Nero, mayor of the nearby village of Brema, added: "This is a tragedy. We are all grieving and awaiting the results of the investigation."
Matteo Sanguineti, director of Fly Emotion, commented: "Our first thoughts are with the woman and her family. We still don't know what happened, I'm shocked. We are cooperating with the authorities to find out the cause. The zipline opened 13 years ago and Over 200,000 people have used it in that time and we've never had an incident."
The zipline stretches over the valley and starts in Albaredo at an altitude of 914 m and ends in Bema at an altitude of 731 m.
